WELCOME TO LAMBETH MINOR HOCKEY'S ONLINE REGISTRATION

Before you begin you will need to create an Account. As you proceed through the process, please pay special attention to the "*" which notes required fields that must be completed in order to move on. Please read all the information on this page and then proceed at the bottom by clicking on the link provided.

For all new participants (that have not played with our Association), you must print off a Registration Form and send a copy of your Birth Certificate, and a copy of your Driver's License (to validate the address with payment) to Lambeth Minor Hockey -- see below for mailing address.

Once you have sent your Registration via mail, please follow-up with an email so that we can track your Registration.

If you are having problems registering your child and they have played with our Association, please send your child's Full Name and Date of Birth to registration@lambethminorhockey.ca.

PAYMENT OPTIONS...
Payment can be made in full by Visa, Mastercard, Cheque, Certified Cheque or Money Order. Post dated cheques will be accepted, please note:
- A minimum of $275.00 is due at the time of Registration
- Fees must be paid in full by August 1st, 2010

Post dated cheques will be accepted for the following dates ONLY:
June 1st, 2010 and August 1st, 2010

Competitive Fee Payments are $185.00 on or before October 12, 2010.
NOTE: This fee will be collected by your team manager for the 2010-2011 season once the competitive rosters have been selected.

Starting April 19th until May 31st, Registration will be open to everyone playing co-ed hockey (Mite through to Major Midget). Any Registrations after May 31st will be considered LATE Registrations. A late Registration Fee will apply ($50.00), and a position on a team will not be guaranteed.

The Volunteer and Fundraising Fee will automatically be put onto your first child’s Registration. As long as you Register all of your children under the same account, you will not be charged twice.

GIRLS (ONLY) HOCKEY REGISTRATION has now been completed.

If you missed our in-person Registration dates, please send us an email to registration@lambethminorhockey.ca and we will get you Registered ASAP!

REGISTRATION WAS NOT/WILL NOT BE DONE ONLINE, BUT MANUALLY FOR GIRLS HOCKEY.

Please DO NOT register girls online unless you are interested in playing Boys/Co-ed hockey (House League or Competitive), or playing in the Tyke hockey program, or Registering for Mite Hockey.

COACHES: If you are interested in coaching, please fill out a coaching application and send it into Lambeth Minor Hockey or drop off to Greg Barber State Farm Insurance office on the Main Street in Lambeth by May 15, 2010. Application forms are available on our website. All selected coaches will require a police check.

FINANCIAL ASSISTANCE: No player will be refused Registration due to financial difficulties. For assistance, a written request must be submitted. If approved, any assistance is based on a house league Registration fee. Speak to any executive member to make alternate arrangements.

Registration fees may be subject to change prior to season due to unforeseen cost increases.

REFUND POLICY: All refunds are subject to an automatic $50 handling fee. NSF cheques are subject to an automatic $30 administrative fee. Replacement of NSF cheques must be by either Certified Cheque or Money Order.

Registration refunds will not be issued until after August 21st and not until the original funds are received (e.g. credit card payments are transferred to the Lancers).

All requests for refunds must be in writing to the Registrar (email will suffice), who must approve them before a refund cheque will be issued by the Treasurer.

If you are withdrawing from the program:
- Full refund available until October 1st minus handling fee
- Oct. 2nd to Nov. 1st – less $100.00 and handling fee
- Nov. 2nd to Nov. 15th - 50% of Registration refunded (less handling fee)
- NO PROGRAM WITHDRAWAL REFUNDS AFTER NOVEMBER 16th
- Please allow 6-8 weeks for processing of refunds
